Le CD Theater Dragon Quest IV (CDシアター ドラゴンクエストIV, CD shiatā doragonkuesuto IV) est une adaptation audio de Dragon Quest IV. L'illustration de la couverture est réalisée par Mutsumi Inomata, et les musiques composées par Koichi Sugiyama.
Un livret de 20 pages accompagne chacun des trois CD composant l'ensemble, illustrants des moments clefs de l'histoire.
